Chris,
What I did was drill and tap a hole through the swing arm and axle shaft.  This seems to lock the axle in nice and tight.

I think i asked this before, but forgot the answer(s) - the bolt that you use to hold your road wheels to your suspension arm, do you just tighten it until it snugs up?
 
even have one come loose or fall out during battle (or fun running)?
