I believe you use two servos, one connected to the up/down and the other
connected to the right/left outputs of the reciever. Each is connected to
a DPDT switch or a MAG style switch pair that serve as the up/down and
left/right switches on the diagram.
You could also do it electronically I expect, but you'd have to interpret
the pwm signals from the receiver.
