We use a pair of flanged-hubs on either side of the wheel. The hub is
connected to the shaft with a pin and connected to the wooden wheel with
3 screws. You can see an example hub at:

The simplest way to make a flanged-hub is to buy some steel 1/2" fender
washers and some steel 1/2" nuts. Drill out the 1/2" nuts to a full
1/2" inside diameter and drill three holes around the outer edge of the
fender washers for some large wood screws. Then, clamp the nut on top
of the washer and braze or weld them together.
